Adult Assertive Community Treatment (ACT) Program. Permanent Position $90-$95/hour Part-Time (28+ hours/week) or Full-Time (40 hours/week).
Join a mission-driven behavioral health team dedicated to serving adults with serious mental illness through an evidence-based Assertive Community Treatment (ACT) model. This community-focused program provides comprehensive psychiatric care, rehabilitation, care coordination, and recovery support for individuals who have not been successful in traditional treatment settings.
As a PMHNP, you will collaborate with an experienced multidisciplinary team to deliver high-quality psychiatric care while helping guide treatment planning and clinical decision-making. Key Responsibilities
--Conduct comprehensive psychiatric evaluations and ongoing medication management
--Develop and oversee individualized treatment plans
--Collaborate with social workers, therapists, nurses, and case managers to coordinate care
--Provide psychopharmacology services, including MAT, clozapine management, and long-acting injectable medications
--Prescribe and manage buprenorphine as clinically appropriate
--Provide clinical guidance, education, and mentorship to team members
--Participate in daily team meetings and interdisciplinary case reviews
--Assist with rotating on-call coverage for crisis support and intervention
--Maintain compliance with New York State regulations and best practice standards
Qualifications
--Master's Degree in Nursing with Psychiatric Mental Health Nurse Practitioner certification
--Active New York NP license
--Active DEA registration
--3,600 hours of clinical practice strongly preferred
--Experience working with adults with serious mental illness preferred
--Strong psychopharmacology and diagnostic assessment skills
--Experience with medication-assisted treatment and community behavioral health settings preferred
--Valid driver's license and ability to travel locally as needed
